Forms and terms

Please note that participation in any PADI Mermaid course requires the completion of a diver medical form. Based on your answers, you may need a doctor’s clearance for diving. This clearance must be provided for the instructor to see before the in-water session of the course as per PADI standards.

Other forms that you need to sign are the Statement of Risk and Liability / Non-Agency Acknowledgment form. In addition, mermaid diving safety rules are reviewed and signed at the beginning of the course.

Contact us if you want to ask about the forms before the course starts. Students of Mer-divers are insured for the duration of the course dives by Divers Alert Network (DAN).
